I never thought about the "hole punching" era but that makes perfect sense to me.

Used to own a copy of this back in the day, and I don't recall it being 3 hole punched through the cover. I remember the inside being 3 hole punched, but the cover looks as though it was punched after-market as it goes straight through portions of the map. The shrink does not look original either, but without seeing it in person, it's hard to tell.

Yeah, the maps were definitely not created with hole punching in mind.

It could conceivably get costly to do new covers, depending on how much needed to be changed. Someone would have to be paid to do new layout work, and possibly text edits to the booklet if necessary to reflect the maps. Each page changed would require a new negative to be made, with corresponding printer plate. The rest would be standard for any printing; four-color cover with one or two passes for the inside (depending whether they did process or spot inks), then single color for each b/w page. It's really about how much prep work needed to be done before the press.

EDIT:

Thinking about it a bit this morning, I could see a possibility where both the maps being punched and then not while the booklet is punched being legit. The first pass would have been done, and then determined in hindsight not to have been a good idea due to the effects on the maps. Then any subsequent printings would be non-punched cover and punched booklet.

Sorta takes part of the usefulness of hole punching away though.
