So I have a question about this fine FGU product....

Does anyone know if every module came with a countersheet? I've searched and can't quite get a good answer.

Not everyone did, but nearly all of them. Off the top of my head, the "Opponents Unlimited" had all the pics on the back cover, not on a countersheet.  This is for classic V&V only, no idea about anything past the 80s...

The first V&V module to come with counters was The Dawn of DNA (FGU #2010). Its copyright/table of contents page says:
"Note that this Villains and Vigilantes modules comes with cut-apart counters to represent the various villains and persons discussed herein. ... These counters have been added to the format of these adventures in response to player suggestions and FGU plans to continue to add counters to new V&V products."

The Dawn of DNA is also the first to give an explicit credit for counter artwork.

All modules produced beforehand were released with NO counters. HOWEVER, some later releases of some of those modules DID include counters. So there are three categories:

1. Never released with counters:
Death Duel with the Destroyers (FGU #2003)
The Island of Doctor Apocalypse (FGU #2004)

2. Originally released without counters, but later releases included counters (I have owned versions of all of these with uncut counters still attached):
Crisis at Crusader Citadel (FGU #2002)
Force (FGU #2005)
Opponents Unlimited (FGU #2007)
Most Wanted Volume 1 (FGU #2008)

3. Always released with counters:
The Dawn of DNA (FGU #2010)
... and everything else up through the end of the 1980's.
