I recently picked up an RPGA1 module at my local Halfprice Books and it was whole-punched (2, not 3; the cover was punched twice).

I'm assuming this was done by the previous owner and didn't come like this, correct?

The booklet was whole-punched, but the detached handout sheet wasn't.

I'm guessing the owner whole-punched the cover, then whole punched the book, then realized they didn't line up, and so he re-whole-punched the cover to line up with the booklet so he could put it in his 80s Trapper Binder, with the handout detached and safe inside a folder pocket.

Correct, did not come hole punched.  It would definitely hurt the value of the book quite considerably.
