I recently acquired a very nice copy of PotVQ, 1st printing (black folder version). It is complete, per the description on the Acaeum. HOWEVER, both copies of the map for Level Four have the numbers present, and both of the maps for Level Five don't have any numbers. Meaning, rather than one level four map for the players, and one map for the DM, there are just two for the DM. And instead of one level five map for the players and one for the DM, there are just two for the DM. Is this a common occurrence? Should I be concerned? Could this be chalked up to being "assembled by hand in someone's basement" as these copies were (I heard that TSR would actually use photocopies sometimes to replace missing pages from copies, so mistakes were made it seems)?

Howdy, i have a 1st print with thee same feature, so i wouldnt worry. I cant remember which map is duplicate and which one is missing but it does look like a "basement assembling" problem.
The only concern i would have if i were in your shoes is that the seller was aware of it and asked a full price... In my case i paid it with a 75% of discount due to this problem!

As few people (if any) are actually going to use this collectible to play the adventure, I wouldn't worry about it at all.  It was a manufacturing "misprint", and you could even hype it up as a "rare error" print.  :)
