I recently acquired a Greyhawk Folio (which I´m overall quite happy with, considering that it is rarely offered in Germany). I tried to discern which printing I have.
It appears to be an second print - black font on the folio cover, copyright note printed on the folio cover, but stamped on the gazeetter cover, art credits only include Dave Sutherland. So far, so clear. But the two maps match the map description of the 4th/5th print, with the copyright info printed on the lower right. The acaeums descriptions mention a copy that matches 3rd print with higher print maps (I assume it should refer to 4th print in the description of the 3rd print). I guess that they could have mixed up parts of various prints as they came along. As the item is overall not that valuable, I don´t think anybody would try and add the maps that may have been lost at some point at a later point?
Do I read that correct that the folio cover from 2nd and 3rd prints are identical?

Furthermore, there is a page torn from the "Playthings" https://en.wikipedia.org/wiki/Playthings_(magazine) magazine (Jan 1980 issue) added to the item, showing the "Gateway to Adventure" ad on one side (pointing to the "HIA trade show, Anaheim" and the "Toy Building Showroom", New York.) The back of this page has a non-RPG related text, the page no. 42 and the magazine title and date. Somebody obviously thought this might be interesting to a collector and put that page in a plastic sleeve...
